When making a Report:

• Convey as much details as possible such as: Who? What? Where? When? Why? How?
• Complaints may be made anonymously or you may request confidentiality.

Note: If you elect to submit your complaint(s) via Internet E-Mail, you must waive confidentiality due to the non-secure nature of internet electronic mail systems.
