If you have
provided information about the quality of your SRM service or needs
assessment, please note the following:
This information
collected is used to evaluate customer service and to determine
future priorities for producing new SRMs. This customer information
(i.e., name, address, phone/fax number(s), e-mail address) will
enable SRMP to follow-up with customers on the issues listed on
the form and to assist NIST in its efforts to provide better measurement
services to science, industry, academia, and government. This information
may also be retained indefinitely as deemed necessary to interact
with customers.
