In June of 1994, a joint investigation conducted by the Federal Emergency Management Agency (FEMA), the Florida Department of Health and Rehabilitative Services, and the FBI, disclosed a scheme to defraud FEMA's Individual and Family Grant (IFG) program. It is alleged that Rafael Maximino Montejo, working as an assistant to the Director of the IFG, submitted and approved for his own benefit fraudulent claims to FEMA pursuant to the 1992 Hurricane Andrew relief efforts. Montejo allegedly received checks from FEMA after filing the false claims. Loss from the known fraud is estimated at over $50,000. In November of 1998, a Federal Grand Jury subpoena was served on Montejo for handwriting examples. Montejo failed to appear for his scheduled appointment and an arrest warrant was issued charging him with contempt. On December 15, 1998, Montejo was indicted and the United States District Court, Southern District of Florida, issued a federal arrest warrant charging him with making false claims against the United States.
