Polaris Industries Announces Recall of ATVs - September 16, 2004 - Polaris is recalling 18,500 "Predator 500" all-terrain vehicles. The front brake lines can crack and leak brake fluid, possibly resulting in loss of braking capability. The recall includes "Predator 500" ATVs from model years 2003 and 2004 with these model numbers: A03GJ50AA, A03GJ50AB, A03GJ50AS, A03GJ50CA, A04GJ50AA, A04GJ50AB, A04GJ50AC, and A04GJ50AD. Suzuki Announces Recall of Four-Wheel Drive Eiger ATVs - September 09, 2004 - Suzuki is recalling about 240 Eiger "QuadRunner" ATVs. Mislocated welds secure the upper front suspension arm mounting brackets to the frame. The mounting bracket could break off during riding, reducing rider control and resulting in loss of control of the ATV. Loss of control could result in a crash and severe personal injury or death. The 2004 model year four-wheel drive Eiger ATVs are affected by this recall. The model numbers are LT-A400FK4 (for automatic transmission) and LT-F400FK4 (for manual transmission). NHTSA Publishes List of June 2004 Recalls - August 03, 2004 - The National Highway Traffic Safety Administration (NHTSA) today released a list of auto safety recalls announced in June 2004 and urged consumers to have the problems fixed promptly. PlayKids USA Announces Recall of Baby Walkers - September 28, 2004 - PlayKids USA is recalling about 1,600 baby walkers. The walkers will fit through a standard doorway and are not designed to stop at the edge of a step. Babies using these walkers can be seriously injured or killed if they fall down stairs. They have model numbers PLK94STP, PLT95STP, PLK2000RC, PLK95, PLK94, PLK98STP, PLK2000 or PLK300 on the package and on a sewn-in label on the seat back. The plastic walkers have eight snap-in wheels, an activity tray with toy wheels, knobs and lights, and a padded fabric seat. The sewn-in seat label also reads "PLAYKIDS U.S.A. Natura Products Downey Inc. Announce Recall of Pacifiers - September 24, 2004 - Natura Products Downey is recalling about 9,400 Jaloma pacifiers. The pacifiers are banned under federal law. They failed federal safety tests, come apart, and can pose a choking hazard to infants and small children. The pacifiers, which measure three inches long and about 1½ inches across the base, are pink, blue, green or white plastic with a yellow nipple. The pacifiers have a circular handle and the shield is labeled "Jaloma." Read more...

Delta Enterprise Corp. Announces Recall to Repair Director’s Chairs for Children - September 23, 2004 - Delta Enterprise Corp. is recalling about 81,000 director’s chairs for children. The chair can inadvertently be misassembled so that the fabric seat can come off the chair’s frame and expose metal support rods. If fabric seat comes off the frame, there is a laceration and fall hazard to young children. The product is a child’s director-style chair, constructed of tubular metal with a canvas seat and back. The chairs feature popular characters such as "The Wiggles," "Dora the Explorer," "Spongebob Squarepants," and "Disney Princesses." The Model Numbers for these recalled units are TC83536WG, TC83531DO, TC83533PS and TC83532SB. The model number can be found on a label on the leg of the chair. Read more...

Reebok International Announce Recall of Toddler’s Athletic Shoes - September 20, 2004 - Reebok is recalling about 140,000 "Iverson/Answer" toddler shoes. The I-3 logo-tag on the tongue of the shoe can be peeled off, posing a choking hazard to young children. The recalled Reebok Iverson mid- and low-style athletic shoes were sold in toddler sizes 2 through 10. The logo-tag on the tongue of the shoe reads "I3/Answer," and on the rear and center of the heel is an "I3" logo. The names "Iverson" and "The Realist" are carved into the sole. The model number is printed on a label on the underside of the tongue. Model numbers included in the recall are: 99553, B99553, 99554, B99554, 105155, B105155, 105158, B105158, 108292, and B108292. Regal Lager Announces Recall to Repair 'Baby Björn' Infant Carriers - September 14, 2004 - Regal Lager is recalling about 49,000 Baby Björn baby carriers. The back support buckle can detach from the shoulder straps, posing a fall hazard to the baby. The recalled infant carriers were sold under brand name "Baby Björn" which is printed on the front of the carriers and on the black molded plastic back support buckle. Model number 1-260 is printed on the care label on the strap of the carrier. The carriers are made of a cotton polyester blend fabric in black with red piping and blue with white piping. "Baby Carrier Active" is written on the packaging and above the warning label on the carrier. Read more...

Hasbro Announces Recall of Monster Rockets - September 09, 2004 - Hasbro is recalling about 230,000 Super Soaker Monster Rockets. The cap on the water tank can unexpectedly and forcibly project off when it is quickly unscrewed from the tank, posing a risk of impact injuries to users or bystanders. In addition, the rocket’s tail can strike a user or bystander on descent, if the rocket is not fully launched, posing a risk of injury. The Super Soaker Monster Rocket is composed of a 7-foot inflatable mylar rocket with a plastic and foam fin section. The rocket has a blue and orange launch base with a water pressure tank attached to one leg of the base. The tank, which has an orange cap, is connected to the pump mechanism, which launches the rocket using pressurized air and water. The water toy has the words "Monster Rocket" printed on the body of the rocket. Read more...

Target Announces Recall of Electric Scooters - September 02, 2004 - Target is recalling 74,811 Leoch electric scooters (also known as "Red Dragon" and "E-Scooter"). Improper wiring can cause a short circuit, posing a fire hazard in the scooter. In addition, inadequate insulation may expose electrical wiring, which poses a shock hazard. The scooters have a seat, two wheels and footrests on each side. A sticker on the left footrest reads, "E-Scooter." On the right footrest is a sticker with a horse-head image. Regal Lager Announces Recall to Repair Stroller Attachments - September 01, 2004 - Regal Lager is recalling 11,500 Lascal "Buggy Board" stroller attachments. The Buggy Board’s red connecting pins can break, causing the board to partially detach from the stroller or carriage, posing a fall hazard to the user. The Lascal "Buggy Board" is a wheeled board that attaches to a stroller or baby carriage, allowing an older child to stand on the board and ride behind the stroller between the arms of a parent or caregiver. The product comes in black, blue, red and techno styles, and has the words "Buggy Board" printed on the top center of the board, with the name "Lasca"” molded into the plastic on the bottom center of the board. Aviva Sports Announces Recall to Repair Astroslide Water Slides - August 31, 2004 - Aviva is recalling 7,500 Astroslide Inflatable Water Slides. These water slides have an excessive length of rope attached to the safety mat that presents a risk of entanglement in the water. This poses a drowning hazard to users. The Aviva Astroslide 17048 is a large, inflatable water slide measuring 5-feet high and 9-feet long. It is used for sliding into a pool or lake. BMW Announces Recall of SlideCarver Scooters - August 27, 2004 - BMW is recalling 161 SlideCarver Scooters. The front column of the scooter does not meet impact resistance standards. The handlebar column can break if the scooter hits an object, which can cause the rider to fall and suffer injuries. The SlideCarver is a three-wheeled scooter. “BMW” and “SlideCarver” are written on the underside of the rider platform of these scooters. Read more...

Meco Corp. Announces Recall of Red Chairs from Children’s Furniture Sets - August 19, 2004 - Meco is recalling about 3,800 Kid’s Essentials five-piece folding furniture set. The red paint on the chair contains excessive lead levels, posing a lead poisoning hazard to young children. The recalled product is a red folding chair with a metal frame and vinyl padded seat back and base. The chair was sold as a part of a five-piece juvenile table furniture set with a green table, blue chair, yellow chair, green chair and red chair. Only the red chairs with model numbers 11-88-3E1 and 11-88S3E1 and date codes H3, B4, D4 or E4 printed underneath the seat bottom are included in the recall. PTI Sports Inc. Announces Recall of Schwinn Toddler Bicycle Helmets - August 19, 2004 - PTI Sports is recalling about 9,000 Schwinn-brand toddler bicycle helmets. The helmets do not comply with impact testing requirements in the Consumer Product Safety Act. These Schwinn-brand Toddler Bicycle Helmets have "SCHWINN" written on the back of the helmets. A white label inside the helmets reads, "Lot# 791913." This number is followed by a date from "12-01-03" through "5-15-04." This label also contains the model number SK103, SK107 or SK108. Nike Announces Recall of Children’s Athletic Shoes - August 18, 2004 - Nike™ is recalling 9,000 Get-Go and Little Get-Go Children’s Athletic Shoes. The 1.5-inch to 1.75-inch rubber tab at the top of the heel can detach from the shoe, posing a choking hazard to young children. The "GET-GO" and "LITTLE GET-GO" children’s athletic shoes come in various color combinations and were sold in toddler and pre-school sizes 2C through 3Y. The Nike Swoosh symbol is on the outside and sole of the shoe. All shoes close with a strap. The style name "GET-GO" or "LITTLE GET-GO" is written on the shoe box. The recalled shoes have one of the following six-digit numbers on a label inside the shoe just above the UPC code: 308638, 308639, 308642 and 308647. Read more...

PJ Sleep Shop Announces Recall to Repair Bunk Beds - August 11, 2004 - PJ Sleep Shop is recalling 337 Wooden bunk beds. These bunk beds have gaps between parts of the upper bunk end structure that violate the federal safety standard because they pose entrapment or strangulation hazards to children. The recalled models of wooden bunk beds are: Twin/Twin #1000; Twin/Double #2000; and Loft Bunk #3000. TOMY Company Announces Recall of Pokémon Plush Toys - August 06, 2004 - Tomy is recalling about 7,400 Pokémon plush dolls, beanbags, and key chains. Tips of sewing needles have been found in the stuffing posing a puncture hazard. There is a production code on the toy’s label that begins with a letter and is followed by two numbers. New York Zion Trading Corp. Announces Recall of Electric Fans - October 06, 2004 - New York Zion Trading is recalling about 2,500 Portable oscillating electric fans. The wiring is undersized and the power plug is not polarized, creating fire and shock hazards. The recall involves 16-inch diameter 3-speed oscillating fans which mount on adjustable stands. "NYZT" appears in the center of the front fan guard. The model numbers FS-1601A and FS- 1607C are found on a silver label on the back of the controls. Recall of Amana, Trane, and American Standard Air Conditioners/Heat Pumps - September 30, 2004 - About 875,000 units are involved in this recall. The units can be a fire hazrd when the filter becomes clogged due to lack of maintenance, severely restricting airflow and resulting in elevated internal temperature. These units are owned mainly by commercial and institutional organizations and are used in the lodging industry, apartments, hospitals, nursing homes and assisted living facilities, schools, and government buildings. Full Speed Ahead Announces Recall of Bicycle Wheel Quick Releases - September 29, 2004 - Full Speed Ahead (FSA) is recalling about 1,375 sets of Scatto bicycle wheel quick releases. An internal part in the lever mechanism can break, reducing clamping effectiveness and rendering the unit inoperable, potentially causing a bicyclist to fall. The Full Speed Ahead Scatto quick release was sold as standard equipment for RD-400 and XC-300 bicycle wheelsets and also separately. Specialized Bicycle Components Announces Recall of Bicycles - September 29, 2004 - Specialized Bicycle Components is recalling about 1,500 Allez Sport, Allez Elite, Roubaix, and Sequoia Bicycles. The handlebar may loosen and turn unexpectedly, which can cause the rider to lose control and fall. Sauder Woodworking Co. Announces Recall of TV/VCR Carts - September 28, 2004 - Sauder Woodworking is recalling 300,000 TV/VCR carts. The carts can tip over and the television can fall off, posing a risk of serious injury or death if the TV and cart fall on a child. The recalled TV/VCR carts are white, light brown and light reddish brown; have decorative hardware and trim; and are about 29 inches wide, 17 inches deep, and 25 inches high. The carts are equipped with a top shelf for a television 20 inches or smaller and a middle shelf for a VCR and a lower storage area. Polaris Industries Announces Recall of ATVs - September 27, 2004 - Polaris is recalling about 12,170 "Sportsman 700 EFI" All Terrain Vehicles (ATVs). The throttle cable may bind when the handlebars are turned full left or full right, resulting in an increase in engine speed and unintended vehicle acceleration. In addition, the fuel line may rub against the vehicle chassis, resulting in a fuel line leak which could be a fire hazard. All model year 2004 "Sportsman 700 EFI" ATVs with model numbers A04CH68CU, A04CH68AU, A04CH68AP, and A04CH68AQ are part of this recall. The model number is on the rear upper frame tube located directly under the right side of the seat. The ATVs have black seats with a grey, red, and black chassis. "700 Sportsman" is prominently displayed on the right and left side of the chassis, body and fuel tank, and "EFI" is displayed on the instrument/headlight pod. Read more...

IKEA Home Furnishings Announces Recall of Glass-Door Wall Cabinets - September 24, 2004 - IKEA is recalling about 25,000 BERTBY Glass-Door Wall Cabinets. Some of these glass-door wall cabinets have the wrong sized screws for the safety bracket, which can result in the cabinet not being properly secured to the wall. The cabinets can fall and injure nearby consumers. The glass-door wall cabinets have "BERTBY," the product article number (380.472.10) and supplier number (14709) written on a label on the underside of the bottom panel or on the top side of the top panel. Bombardier and John Deere Announce Recall of ATVs - September 22, 2004 - Bombardier Recreational Products and John Deere are recalling about 23,000 All Terrain Vehicles (ATVs). The front brake hose can be pulled out of its retaining brackets on either side of the ATV by foreign objects. This can cause the brake hose to wear by rubbing on the inner wheel or shock absorber spring seat, ultimately causing a brake fluid leak resulting in a complete loss of front braking capacity. This can lead to an increase in the braking distance and possible collision with bystanders, fixed objects, or other vehicles, causing serious injury or death. The 2003, 2004, and 2005 year models of "Traxter," "Traxter MAX," and "Quest" Bombardier ATVs are involved in this recall. In addition, the 2005 year models of "Buck" and "Trail Buck" John Deere ATVs are involved in the recall. These 4-wheel ATVs are red, green, yellow, or black in color. Read more...

Eddie Bauer Announces Recall of Ebtek Brand Backpacks with Stools - September 22, 2004 - Eddie Bauer is recalling about 300 Ebtek brand backpacks with stools. The stool could collapse and cause the person using the stool to fall. The stool does not meet the firm’s strength requirements. Performance Inc. and Supergo Inc. Announce Recall of Bicycle Handlebars - September 22, 2004 - Performance Inc. and Supergo Inc. are recalling about 300 Forte Flyte OS" and "Weyless CF200" carbon bicycle handlebars. The handlebars can develop cracks that may not be visible, which can cause the handlebar to break without warning, resulting in serious injury or death. Both handlebars are gray. The Performance Forte Flyte has printing along the top of the handlebar. On the left front is "Forte" in red, and on the right front is "Flyte." On the right top is "ACT" in large white letters and "Advanced Carbon Technology" in smaller letters. The Supergo Weyless handlebar says "Weyless" in white and has a red oval with "Full Carbon Composite" printed in white. On the left side is "CF" in red, "200" in white, and "Hi-Pressure Solid Compaction" in white. The Performance handlebars were sold separately, not as part of a bicycle. Wild Country Ltd. Announces Recall of Helium Carabiners - September 21, 2004 - DMM Engineering is recalling about 1,000 Wild Country-brand Helium carabiners used in rock climbing. The carabiner gate may come open under a heavy load, which will significantly reduce the strength of the carabiner. The carabiner could break if the climber falls, posing a risk of serious injury or death to the climber. These are Wild Country-brand carabiners sold under the following model names: Helium Dyneema, Helium DYN QD 5 X 13, Helium Clean Wire, and Oxygen- Helium. Gruga U.S.A. Announces Recall to Repair Office Chairs Sold at Staples Stores - September 21, 2004 - Gruga U.S.A. is recalling 18,000 Executive Office Chairs. The legs on the base of the chair can break, posing a risk of injury to the user. The recalled executive office chairs include the 795-0115 model with black leather and the 795-0228 model with black fabric. Underneath the seat cushion the name "Novimex Fashion Ltd." can be found on the large label and the model numbers and date codes can be found on a smaller label. IMI Cornelius Announces Recall to Repair Frozen Carbonate Drink Dispensers - September 17, 2004 - IMI Cornelius is recalling about 990 Frozen Carbonate Drink Dispensers. The motor drive can overheat and cause the liquid contents to become hot. If this happens, the dispenser’s nozzle can break and expose consumers to the hot liquid, posing a risk of burns. The units contain two or four dispensing valves and come in various color combinations. Arctic Cat Announces Recall of Certain 2004 Snowmobiles - September 16, 2004 - Arctic Cat is recalling 5,193 snowmobiles. The red plastic skis used on certain 2004 Firecat and Sabercat snowmobiles could crack or break as a result of off-season exposure to ultraviolet sunlight. Ski failure could lead to loss of control that could result in severe injury or death. All 2004 Arctic Cat Firecat and Sabercat snowmobiles with red plastic skis are included in this recall. NCR Corp. Announces Recall of NCR Universal Inkjet Refill Kits - September 15, 2004 - NCR is recalling about 78,000 universal inkjet refill kits. The product does not have required child-resistant packaging or required warning labels. The product contains ethylene glycol, posing an ingestion hazard to young children. The recalled kits were sold in a clear plastic package with color or black ink. The black ink kits have model numbers 943264 and 999292 and the color ink kits have model number 999289 printed on the side panel above the UPC code. The two black inkjet sets contain four plastic bottles of black ink, two plastic bottles of cleaning fluid and one toolkit. Lexmark Announces Recall of Laser Printers - September 07, 2004 - Lexmark is recalling 39,431 laser printers. These printers can short-circuit, posing an electrical shock hazard to consumers. The recall includes Lexmark, Dell and IBM brand laser printers. The brand name and model number for the Lexmark and IBM laser printers can be found on the front of the printer. For Dell laser printers, the brand name is on the front of the printer and the model number is inside the front cover. Pelagic Pressure Systems Announces Recall of SCUBA BC Inflators - September 02, 2004 - Pelagic Pressure Systems is recalling about 11,600 Oceanic-brand Reliant Buoyancy Compensator (BC) Inflators and AERIS-brand AW3 BC Inflators. The Power Inflator Buttons can stick, which can cause uncontrolled inflation of the BC. Halcyon Manufacturing Announces Recall of SCUBA BC Inflators - September 02, 2004 - Halcyon SCUBA is recalling about 4,000 Buoyancy Compensator (BC) Inflators. The SCUBA BC bladder can have a slow leak because of imperfections within the machining of the stainless air barrel of their inflators. This can cause unexpected buoyancy problems with divers, possibly resulting in decompression sickness. Halcyon is recalling all stainless steel power inflators. IBM Announces Recall of AC Adapters for Notebook Computers - September 02, 2004 - International Business Machines (IBM) is recalling 225,000 AC Power Adapters. The adapters can overheat, cause damage to the circuit board and melt through the housing, which poses a fire and electrical shock hazard to consumers. The recalled 56W AC power adapters were shipped primarily with IBM ThinkPad i Series, 390 and 240 Series, and s Series notebook computers. The adapters have three hollow pins at the AC inlet and part number 02K6549 on a label on the top of the unit. The IBM logo appears on top of the unit. Theraline Announces Recall of Maternity and Nursing Pillows - September 01, 2004 - Theraline is recalling about 1,000 Big V Maternity and Nursing Pillows. If infants are placed on these pillows and left unattended, there is a risk of suffocation since the pillows are loosely filled with plastic beads. Boston Billows Announces Recall of Nursing Pillows - September 01, 2004 - Boston Billows is recalling about 8,000 Nursing Pillows. If infants are placed on these pillows and left unattended, there is a risk of suffocation since they are loosely filled with plastic beads. Whirlpool Announces Recall of Cooking Products - August 26, 2004 - Whirlpool is recalling 24,500 freestanding ranges, double ovens, and oven-microwave combinations. All of the appliances use faulty wiring, which can overheat, melt through the insulation and cause a short circuit. GE Announces Recall of Ranges and Wall Ovens - August 26, 2004 - General Electric (GE) is recalling 28,300 GE, Hotpoint, Kenmore, and Americana brand freestanding electric ranges and double wall ovens. The ranges and wall ovens have faulty wiring, which can melt and cause the oven to short circuit. The appliance can then stop working and pose a shock hazard to consumers. Apple Announces Recall of PowerBook Computer Batteries - August 19, 2004 - Apple Computer is recalling 28,000 rechargeable batteries used in 15-inch PowerBook G4 computers. An internal short can cause the battery cells to overheat, posing a fire hazard to consumers. The recalled batteries are used with the 15- inch PowerBook G4 (Aluminum) laptop computer. The batteries, which contain cells manufactured during the last week of December 2003 only, have the model number A1045 and serial numbers that begin with HQ404, HQ405, HQ406, HQ407, or HQ408. Belkin Corp. Announce Recall of Rackmount Uninterruptible Power Supplies - August 17, 2004 - Belkin Corp. is recalling about 1,200 rackmount uninterruptible power supply (UPS) units. The UPS outputs were wired incorrectly creating a reverse polarity configuration and a potential electrical shock hazard to consumers. The recalled Belkin OmniGuard Rackmount UPS units are used to provide voltage regulation, surge suppression, and back-up power for electronic equipment. Vaxcel International Announces Recall of Ceiling Fans - August 13, 2004 - Vaxcel International is recalling 1,200 ceiling fans. These ceiling fans were assembled without the proper wire insulation sleeving, which could result in exposed wiring. Consumers could receive an electrical shock during installation or removal. These down-rod-mount, dual-motor, 36-inch ceiling fans were sold in chrome, stone white, brush nickel, polished brass, antique brass, or weathered patina finishes. Model number 355-6645 is printed on the box and on a label located on the central housing of the ceiling fan. The brand names of the fans are “Aire Tek” and “Turn-of-the- Century Apollo Series” which is written on the packaging only. OTT-LITE Technology Announce Recall of '10,000 Lux Light Box' That Simulates Natural Sunlight - August 11, 2004 - OTT-LITE Technology and Natural Options Inc. are recalling 226 "10,000 Lux" Light Boxes. Overheating around the socket (where the bulb fits into the light box) can result in melted plastic and smoke, posing a possible fire and burn hazard. Frito-Lay Recalls Tostitos Monterey Jack Queso Dip Due to Health Risk - September 24, 2004 - Frito-Lay announced the voluntary recall of Tostitos Monterey Jack Queso Dip due to the potential for the product to be underprocessed. While no illnesses have been reported, there is a possibility that the product could be contaminated with harmful microorganisms. The company is now retrieving the product from its distribution system and store shelves. The recall extends to only Tostitos Monterey Jack Queso Dip distributed nationally. This is a white cheese dip in a 15 ¼ ounce glass jar with a blue label. The specific jars have a freshness date of April 8, 2005 through June 16, 2005, which is printed on the shoulder of the jar below the lid. Iowa Firm Recalls Lunchables Due to Mislabeling - September 23, 2004 - A Kraft Foods Global, Inc. establishment in Davenport, Iowa, is voluntarily recalling approximately 1,000 packages of ham and swiss cheese ready-to-eat complete meals (Lunchables) due to mislabeling. Iowa Firm Recalls Pork Products That May Contain Foreign Material - September 17, 2004 - Sioux-Preme Packing Company, a Sioux Center, Iowa, firm, is voluntarily recalling approximately 1,110 pounds of pork products that may contain foreign material. The products subject to recall are 110 pork shoulder butts that may contain small electronic transponders. The devices were inserted in the shoulders of the animals at a livestock production facility and the animals were inadvertently shipped to slaughter. FDA Issues Public Health Advisory on Vioxx as its Manufacturer Voluntarily Withdraws the Product - September 30, 2004 - The Food and Drug Administration (FDA) today acknowledged the voluntary withdrawal from the market of Vioxx (chemical name rofecoxib), a non-steroidal anti-inflammatory drug (NSAID) manufactured by Merck & Co. FDA today also issued a Public Health Advisory to inform patients of this action and to advise them to consult with a physician about alternative medications. Merck is withdrawing Vioxx from the market after the data safety monitoring board overseeing a long-term study of the drug recommended that the study be halted because of an increased risk of serious cardiovascular events, including heart attacks and strokes, among study patients taking Vioxx compared to patients receiving placebo. The study was being done in patients at risk of developing recurrent colon polyps. Read more...

Fort Dodge to Comply With FDA's Request to Recall ProHeart 6 Injectable Heartworm Product from the Market Due to Serious Health Concerns - September 03, 2004 - Fort Dodge Animal Health, of Overland Park, Kansas, at FDA's request, has agreed to immediately cease production and recall its heartworm medication ProHeart®6 from the market until the FDA's concerns about adverse reaction reports associated with the product can be resolved. ProHeart®6 is an approved injectable sustained-release heartworm prevention product for dogs. FDA is also advising veterinarians to avoid administering this product to dogs until further notice.
