Information Needed

Information concerning a federal violation, or which may be useful to the FBI, can be forwarded to the appropriate FBI office.  In order for the FBI to act on your information, the following details should be provided. (as much as possible):

  • Subject (person)
    • Name
    • Alias(es)
    • Address
    • Phone Number
    • Physical description
      • Height
      • Weight
      • Age
      • Race
      • Distinguishing features
  • Type of crime that has been, is being, or will be committed
  • Location of crime
  • Approximate date or period when crime occurred or will occur
  • Approximate dollar value of injury or loss (if known)
  • Your source of information
    • Were you a witness to the incident?
    • Or, did you hear about it from someone else?
      • If so, from whom?

Email

You may provide information to the Springfield Field Office by sending e-mail to springfield@fbi.gov. If possible, please include your address and telephone number in addition to your e-mail address. Due to the high volume of mail we receive, we cannot respond to all messages.

If the information you wish to provide pertains to an emergency situation, please contact us at (217) 522-9675. Although we check our e-mail frequently, we do not consider it to be an alternative to telephonic contact in emergency situations.

Please place all the information you wish to include in the body of the message. We ask that at least, initially, you do not include attachments because varying file types, encoding/compression schemes, and operating systems sometimes makes viewing them difficult.

Fraud/Scams

Health Care Fraud

Help us stamp out health care fraud by reporting fraudulent activity to the task force. Complaints can be filed by calling the task force toll free hotline at telephone number (888) 557-9503. You don't have to give your name when you file a complaint. However, it would greatly assist the task force if you would leave your name, address and telephone number, so task force members can conduct a complete and thorough interview to ensure your complaint receives prompt attention.

Because of the complexity of these cases, the investigations may take many months or even years to complete. Also, in certain instances involving very sensitive matters, it may not be possible to give out information during the course of the investigation.

As a consumer you are often in the best position to tell the authorities if you believe you are the victim of fraud and abuse. Let's work together to stop health care fraud.
