Flag Request Form

Flag Request Information

If you are choosing to have a dedication message, please be aware the message must comply with the following format:

“At the request of the Honorable Mark E. Amodei, Member of Congress, this flag was flown on behalf of [blank], in [celebration/recognition/memory/honor…] of [blank]. ”  

Additionally, please note our office will not be able to process your request until after we receive your check. All postal mail sent to my offices must be scanned for security purposes, which means it will take about two weeks for your check to reach our office. Once we receive your check, we will be happy to submit your request to the Capitol flag office. Please note the Flag Office requires two weeks processing time after we submit your request before they are able to fly the flag over the Capitol.
