If you are experiencing problems with the Social Security Administration (SSA) and desire my assistance with issues such as:
- Retirement benefits;
- Non-receipt of monthly benefits, overpayment or under payment of benefits;
- Disability or SSI (check the status of your claim, delay in processing or need expedited processing);
- Lost social security card, obtaining a replacement card;
- Medicare card, premiums or eligibility.
Please complete our required authorization form and provide a copy of any documents which substantiate or are important to the case (medical records related to claims are not required unless they are not already on file with the SSA). The form should be completed in detail and include your Social Security number, signed by the individual concerned, and should be either faxed or mailed to my Miami Gardens district office at:
