Thank you for your interest in the U.S. Service Academies. As your 15th District Representative in the U.S. House of Representatives, each year I have the privilege of nominating a limited number of people to four of the five service academies. The honor of attending a service academy comes with an obligation and commitment to serve in the military for a minimum of five years upon graduation. In order to be considered for a service academy nomination, please download and complete this 2016 Service Academy Nomination Application and return it with all supporting documentation to the Hayward District Office no later than 5 PM, October 21, 2016.  If you have any issues downloading the application please call our office at 510-370-3322.

Please note that transcripts must be transmitted directly from your school registrar and test scores must be transmitted directly from the relevant testing agency. For ACT scores please use reporting code 7254 and for SAT scores please use reporting code 4475.
