Please read all the directions carefully.  Failure to submit any part of the application will disqualify you from consideration.

The online application must be completed and submitted in one sitting.  Once you have clicked submit, you cannot return to the application form to make modifications.  You may use simple html in the free form fields to make it easier to read.  Once you have submitted the online form you still must submit several more items via mail.

Please submit physical copies of the additional required materials to complete the application process no later than Wednesday, September 14, 2016 at 5:00 P.M. Those additional materials should be submitted together in a single envelope to:

Your application will not be considered complete until we receive the following hard copy materials:

1) Personal Recommendation Forms - Candidates must submit three (3) personal recommendations. Ask your guidance counselor or high school principal to complete one. Choose two other references, such as a math or science teacher, coach, employer or religious or community leader. The personal recommendation form is attached. In lieu of the form you may submit a written letter of recommendation, but the letter must include the general information requested on the form. The Academy Selection Board will review only three (3) recommendations. All references should be from non-family members.

2) Academic Transcript - The official transcript must be in a sealed envelope with a signature across the seal. If you have already graduated from high school, in addition to your high school transcript, please send your college transcript and a current class schedule. Please include a copy of your attendance and disciplinary record.

3) Record of Attendance - Please have your school's guidance counselor fill out the record of attendance and disciplinary record form.

4) Standardized Test Results - Please include a copy of your ACT or SAT scores in your application packet. They may appear on your sealed transcript or in a sealed envelope from your guidance counselor. If you plan to take the October SAT, your scores will not reach our office in time for consideration. When requesting your scores, please reference SAT# 2809 and ACT# 7119 for the Office of Senator Isakson.

5) Photo - Please provide a wallet sized photo with your name on the back.

6) Candidate Fitness Test - Please provide a copy of your CFT. Copies may be faxed to my office at (770) 661-0768 by academy representatives if you have taken the test at a Summer Seminar. If you did not attend a Summer Seminar, the test may be administered by liaison officers, your high school coach or physical education teacher. Deadline - Completed application packets must be received by Senator Isakson's Atlanta office by Wednesday. September 14. 2016, by 5:00 P.M. No exceptions will be made.  Please print a copy of your application before you hit submit and include it with the other materials that must be mailed to the office. We will send an e-mail confirmation once we receive all of your hard copy materials. Due to the volume of applications we receive, it may take up to two weeks from the date of receipt to update the system and send the confirmation.
