Residents of Indiana’s 2nd Congressional District can contact my office for help resolving issues with a federal agency, such as obtaining Social Security, Medicare, or veterans benefits. To request casework assistance, please complete the form below, print and sign it, and mail it to my office.

To request casework assistance, please complete the form below. When you have included all relevant information, click the button to generate a printable form. You must print, sign, and mail this form to my office in order for my staff to assist you.
Please provide all pertinent information and claim numbers in your correspondence – including:
- Your Social Security number for a case involving Social Security;
- Your Veterans Affairs claim number for a case with Department of Veterans Affairs;
- Your taxpayer identification number (Social Security number, if individual) for an Internal Revenue Service problem, etc.;
- Your address, home phone number, and daytime phone number (if different than home) so that we can obtain any additional information from you that might be necessary; and
- Copies of any related documents or correspondence that you may have from the agency involved.
