My district offices are available to assist you with all federal issues and with any difficulties you may be experiencing with government agencies. Please contact one of my district offices in Evanston or Chicago by telephone, mail, or email if you need help with:

Under the provisions of the Privacy Act of 1974, I need to have formal authorization from you before making an inquiry to a federal agency on your behalf. This law was enacted to protect individuals from unauthorized disclosure of personal information without their consent. Therefore, in order for our office to effectively advocate on your behalf, we require you to fill out and return a privacy waiver. For your convenience, a privacy waiver can be found here. The form must be signed by the person whose case is in question.
When returning the privacy waiver, it would also be very helpful if you would include all pertinent information such as your name(s), mailing address, telephone number(s), Social Security Number, Medicare number, Alien number, case number, etc. Also please include documentation from the agency including correspondence, bills, and other information relevant to your case. It is also most helpful if you include a letter describing the situation and explaining your request. Please indicate if it is acceptable to forward your letter to the relevant agency.

The rules of the House of Representatives prevent me from intervening in the proceedings of any court of law (including appeal proceedings), since these are activities of the judicial branch of government. If you are having a legal problem involving the courts, you may need to consider help from an attorney.
