Help with a Federal Agency

One of the most important parts of my job as your representative in Congress is to help you communicate with federal agencies. Although I cannot override the decisions made by a federal agency, I can often intervene on your behalf to answer questions, find solutions, or just cut through the red tape. 

I can make inquiries as to the status of your case, obtain additional information, or direct you to appropriate resources or services. My role is to assure you a fair and thorough review of your case and to seek a timely response from a federal agency. 



Are You Eligible for Assistance?
  1. Are you a resident of the 12th District of Ohio? You can check by entering your zip code on House.Gov.
  2. Does your case involve a federal agency? Click here to view a list of federal agencies on USA.gov.
  3. Does this case primarily involve you or your dependent? Privacy laws can limit your ability to represent another individual or group.

If you answered “yes” to all these questions, please complete this Privacy Release Form and mail it to our Central Ohio Office.

Can I get my case expedited?
  • It is rare that a case can be “sped up” for any reason other than extreme hardship. Please note that all time-sensitive matters should be addressed by calling my District office at (614) 523-2555 or toll-free 1 855-PAT-OH12. My staff will discuss your situation and help you determine the proper strategy. 

How do I ask for help?
  • While my staff and I cannot guarantee you a favorable outcome, we will do our best to help you receive a fair and timely response from the federal government. If you need assistance, please complete a Privacy Release Form and mail it to my Central Ohio office:

How do I know I am completing the correct form?

  • Once your information has been received and processed, you receive a confirmation letter or phone call informing you of the staff member that will be assisting with your case.

Ohio Agency Inquiries
  • My office can make inquiries with federal agencies, however if you have an issue with a state agency or policy you will need to contact your State Representative or State Senator.  You can find out who your representatives are at http://www.ohiohouse.gov.
