Help with Federal Agency

Residents of the 21st Congressional District of Texas can contact me for assistance in dealing with Federal agencies. In order to better serve you, this form will generate a printable page that you should sign and mail to my office.

One of the most important things I do as a U.S. Representative is help people with federal problems. Although I cannot override the decisions made by a federal agency, I can often intervene on a person's behalf to answer questions and help find solutions. However, regardless of which agency you need help with, I need a completed privacy authorization form to comply with the provisions of the Privacy Act.  If you need help with the IRS, please click here to fill out this specific printable form. For all other agencies, click here for a printable version of the privacy authorization form. Then fax or mail your completed form to the address listed under the district office from which you are seeking assistance. IRS and privacy release form also available in Word format.

If you are having a personal emergency with a federal agency, please contact your nearest district office directly.
If you can't get an answer from a federal agency in a timely fashion, or if you feel you have been treated unfairly, my office may be able to help resolve a problem or get you the information you need. While we cannot guarantee you a favorable outcome, we will do our best to help you receive a fair and timely response to your problem.

Please include all pertinent information and claim numbers in your correspondence.

Please Note:

The Privacy Act of 1974 (5 U.S.C. § 552a) requires that Members of Congress or their staff have written authorization before they can obtain information about an individual's case.
We must have your signature to proceed with this type of request.
