If you would like assistance with a federal agency, whether you have a problem with the Social Security Administration, the Citizenship and Immigration Services, the Department of Veterans Affairs, or the United States Postal Service, Congressman Lieu's office may be able to help. 

According to the Privacy Act of 1974, in order for Congressman Lieu to assist you, written authorization is needed.  Please complete the Privacy Release Form and return it to Congressman Lieu's Los Angeles office at 5055 Wilshire Blvd. Suite 310, Los Angeles, CA 90036 or fax it to (323) 655-0502.
