I Need Emergency Assistance

Please fill out the Passport Intake form with your signature and fax it to our Plano office at 469-304-0392. On the cover sheet please write “Passport – Urgent” and we will begin to process your request as soon as possible.


Help with a Passport
Please contact my District Office in Plano for assistance at 469-304-0382. 

Be prepared to provide the following information to our passport caseworker:

  • Full Name
  • Date of Birth
  • Social Security Number
  • Date of Departure
  • Passport locator number

Important Passport Information

The Intelligence Reform and Terrorism Prevention Act of 2004 requires all travelers – U.S. citizens and foreign nationals alike to present a passport or other document (or a combination of documents) that denotes their identity and citizenship when entering the United States.

Congress amended portions of the Act in 2006. The Western Hemisphere Travel Initiative (WHTI) is the Administration’s proposed plan to implement this mandate.

Accordingly, WHTI requires all people traveling to and from the United States, Canada, Mexico, Central and South America, the Caribbean, and Bermuda to present a passport or other accepted document that establishes the bearer’s identity and nationality to enter or re-enter the United States. The goal is to strengthen border security and facilitate entry into the United States for U.S. citizens and legitimate international travelers.
