How to Request a Flag Flown Over the Capitol
To help commemorate special occasions such as birthdays, anniversaries, graduations, or personal accomplishments, you may have a flag flown over the Capitol - and my office is happy to submit these flag requests on your behalf! However, due to the volume of requests, we cannot provide these flags for free.
If you would like to order a Flag, please complete the Flag Request Form below - this is necessary in order to calculate the price and shipping costs.
Please note that there are two forms of payment: online and by check.
To pay online, complete the Flag Request Form.
If you prefer to pay by check, you must first fill out the Flag Request Form and calculate the price. After you do this, print out your form and mail it to my Washington, D.C. office with your check. Checks should be made payable to "Sam Johnson's Office Supply Account."
