While we cannot guarantee a favorable outcome, we will do our best to help you receive fair and timely attention to your concern. Authorization Form
Before we proceed, the Privacy Act of 1974 requires that Members of Congress or their staff members have written authorization before they may obtain information about an individual's case.

Please fill out and sign a Privacy Release form by downloading the General Intake Form. For immigration cases, you must use the USCIS Intake Form.
