ACADEMY NOMINATIONS

Note: this application is for the year November 1, 2016. Due to Hurricane Matthew, the deadline has been extended to November 22, 2016.

Thank you for your interest in the U.S. Service Academies.

As your Representative in the U.S. House of Representatives, each year I have the privilege of nominating a limited number of people to four of the five service academies. The honor of attending a service academy comes with an obligation and commitment to serve in the military for a minimum of five years upon graduation.

To apply for a service academy nomination from my office, please complete the form below and submit with the other required items to the Concord District Office by 5:00 p.m. on Tuesday, November 22, 2016.

Required Items:

1. Completed "Service Academy Candidate Information Sheet"

2. An essay that summarizes your reasons for wanting to attend a Service Academy. (500 word count max; 12 point font, Times New Roman.)

3. Official high school transcript (sealed by your school).

4. Official SAT/ACT Scores (sent directly from SAT [5314] or ACT [7586] or notated on your sealed official high school transcript). If you submit your scores using the SAT/ACT 4 digit codes, it is your responsibility to confirm that the scores have been received. We require scores from at least one test but not both.

5.Three (3) letters of recommendation from non-relatives, at least two of which must be from teachers or school officials. These should be in envelopes sealed by the recommender. More than three letters of recommendation will not be accepted.

6. A recent photograph, with your name clearly printed on the back.
