Help with Federal Agency

If you are having problems with a federal agency or have questions about services the government provides, my staff in North Carolina may be able to help you. They can direct you to the appropriate office, provide you with necessary instructions, phone numbers or materials, and find out the status of an application for you.

My office resolves cases involving federal agencies ranging from the Social Security Administration to the VA. If you are having difficulty contacting or resolving a problem with a federal agency, please follow the instructions below to request assistance.

Instructions

Please Note:

Due to the Privacy Act of 1974 (5 U.S.C. § 552a) my office will need a signed letter/privacy release form from you, giving my office permission to look into your case. No inquiries from my office can be made without such written consent. An e-mail will not suffice as a release.

We must have your signature to proceed with this type of request.

Please include all pertinent information and claim numbers in your correspondence—such as:

  • Your Social Security number for a case involving Social Security
  • VA claim number for a case with Department of Veterans Affairs
  • Taxpayer identification number (Social Security number, if individual) for an Internal Revenue Service problem
  • Your physical and mailing address, home phone number and daytime phone number (if different than home) so that we can obtain any additional information from you that might be necessary
  • Copies of any related documents or correspondence that you may have from the agency involved

*Please note that some agencies may request additional documentation.

In order to better serve you, this form will generate a printable page that you should sign and mail to my office:
