Thank you for your interest in the U.S. Service Academies. As your 13th District Representative in the U.S. House of Representatives, each year I have the privilege of nominating a limited number of people from Florida’s 13th Congressional District to four of the five service academies. The honor of attending a service academy comes with an obligation and commitment to serve in the military for a minimum of five years upon graduation. To request a Military Academy Nomination form my office, please complete the form below and send it, along with all required supporting documents, to my St. Petersburg office located at 425 22nd Avenue North, St. Petersburg, FL 33704. The form must be received no later than the close of business on Friday, October 7, 2016.
Include the completed application with the following items and mail or deliver all items to one of my district offices:
- Current photo
- High school transcript
- American College Testing Program (ACT) results and/or College Board Scholastic Assessment Test (SAT) results
- Resume of extracurricular activities
- An essay stating your motivation and why you want to attend a service academy (500 words or less)
- Minimum of three letters of recommendation (examples include: Teachers, Coaches, Clergy, Employers, Scout Leaders)
