In order to receive a flag, you need to complete the form below and enclose a check or money order for the complete amount. Due to my upcoming retirement at the end of the year, I will be unable to assist with requests for flags after December 19, 2014. For requests received before this date, please make your check or money order payable to "Keeper of the Stationary," and mail it, along with the completed form, to:

Currently, mail being delivered to the United States Congress is delayed up to 4 weeks because of extra precautions being taken to prevent the delivery of biological agents, so please allow 4-6 weeks for delivery.
