Military Academy Nominations

Instructions

Thank you for your interest in the U.S. Service Academies. As your 1st District Representative in the U.S. House of Representatives, each year I have the privilege of nominating a limited number of people to four of the five service academies. The honor of attending a service academy comes with an obligation and commitment to serve in the military for a minimum of five years upon graduation. To request a Military Academy Nomination from my office, please complete the form below or you may provide all of the required information (listed below) by mailing it to my Tulsa District office (postmarked no later than October 21st) or you may bring to the Tulsa District office during regular working hours on or before the 21st of October.  Just select one application process, either the on-line or paper application process.  If you mail or bring in your nomination application all-supporting documents (including letter(s) of recommendation, transcripts, copies of test scores, etc.) must be in the single envelope you provide. It is the candidate’s responsibility to ensure their application packet is complete at the time of its submission.

Applications and all supporting documents must be submitted at the same time (online) or in a single envelope and are due (or postmarked) no later than October 21st.  No exceptions will be made.  Failure to follow the application process could disqualify you from consideration.

An appointment to the Service Academies is based on a desire by the candidate to devote a lifetime of military service and implies recognition by the appointee of an obligation to the government to devote him / herself to a military career. Are you interested in an appointment on that basis?
