Whether you’re looking for more information about applying for a Medicare health plan or are having an issue with receiving your benefits, our office stands ready to assist you. Please be aware that although a United States Congressman does have authority in legislative matters, the power to change decisions made by a government agency lies within the executive branch of government.

Q: WHERE CAN I FIND INFORMATION ON MY HEALTH CARE OPTIONS WITH MEDICARE, HEALTH CARE PROVIDERS, OR PRESCRIPTION DRUG PLANS?

Q: I COMPLETED MY CASEWORK AUTHORIZATION FORM FOR CONGRESSMAN BOEHNER’S OFFICE TO ASSIST ME WITH MY ISSUE. NOW WHAT?

A: In order for Congressman Boehner’s office to make an inquiry to a federal agency on your behalf, we first need your permission.  If you’ve completed a CASEWORK AUTHORIZATION FORM, here’s what you can do next (please choose one):

  • Submit your Casework Authorization Form by fax.
  • Submit your Casework Authorization Form by mail.
  • Submit your Casework Authorization Form in person by visiting one of our offices.

Q: AM I ELIGIBLE FOR GOVERNMENT BENEFITS AS A SENIOR? HOW DO I CHECK MY ELIGIBILITY?

A: You may check your benefits eligibility by CLICKING HERE or visiting www.benefits.gov.  If you’re having issues with determining your benefits, please fill out this CASEWORK AUTHORIZATION FORM and contact our office to see if our casework team can assist you.

Q: HOW DOES THE PATIENT PROTECTION AND AFFORDABLE CARE ACT (COMMONLY KNOWN AS ACA OR OBAMACARE) IMPACT MY MEDICARE COVERAGE? DO I NEED TO ENROLL IN AN OBAMACARE PLAN?

A: If you have Medicare (Original Medicare or a Medicare Advantage Plan (like an HMO or PPO), you are considered ‘covered’ by Medicare for the purposes of the insurance mandates and do not need to enroll in an ObamaCare plan.  If you only have Medicare Part B, then you are not considered to have minimum essential coverage.  This means you may have to pay a penalty.

Q: I LOST MY MEDICARE CARD. HOW DO I GET A NEW ONE?

A:  Protect your Medicare card just as you would a credit card, as your Medicare card in the wrong hands can be used to submit fraudulent claims.  If you lose your Medicare card, you may take the following steps to obtain a new card:

Q: I RECENTLY MOVED TO ANOTHER STATE. HOW DO I UPDATE MY ADDRESS WITH MEDICARE?

A: To protect you and your benefits, please let your local Social Security office know that your address has changed BY CLICKING HERE or calling 1-800-772-1213.

Q: ALTHOUGH I'M ABLE TO WALK, I HAVE A DIFFICULT TIME WALKING LONG DISTANCES. WOULD MEDICARE ALLOW PAYMENT FOR A WHEELCHAIR OR POWER OPERATED VEHICLE (POV)?

A: Medicare wheelchair and POV coverage guidelines indicate that in order to allow payment for a wheelchair, a person with Medicare must be bed or chair-confined without the use of a wheelchair. Furthermore, all of the following criteria must be met:

  • You require a wheelchair to maneuver in your home;
  • You cannot operate a manual wheelchair;
  • You can safely operate the controls of a POV;
  • You can transfer safely to and from a POV and have adequate trunk stability to safely ride in a POV
  • You need a prescription from your physician prior to purchasing the POV, and the physician and supplier must complete a Certificate of Medical Necessity (CMN).
