Thank you for expressing an interest in ordering a flag from our office. Dave Camp can have a flag flown over the United States Capitol for you, a friend or in honor of a special occasion. An order form is available for you below. If you are interested in ordering a flag that has been flown over the Capitol, please fill out, print, and send a copy of the order form to my Washington, D.C. office. An official certificate will be included with your flag listing the date it was flown and any other information you wish to be printed on the certificate.

Your check must be included with the order form and made payable to Congressman Dave Camp Office Supply Account. All prices include shipping and handling. Please send the form and check to the Washington D.C. address listed at the bottom of the order form.
