Request Congressional Recognition
Congressman Camp's office sends special Congressional recognition certificates to young people in the Fourth Congressional District of Michigan. For example, attaining the rank of Eagle Scout and the Girl Scout Gold Award, or equivalent.

Certificates to congratulate a citizen of the Fourth District who has earned public distinction for outstanding public service to the community are also available.  You may submit your certificate request by filling out the form below and submitting it to Congressman Camp's office, or you can mail or fax a request to:
