U.S. Academy Nominations
Each year it is my privilege to recommend a select few of Northeast Indiana's best high school graduates for admission to the nation's service academies -- Army, Navy, Air Force and Merchant Marine. When applying for a nomination to a service academy, an applicant must apply to both the respective academy and through my congressional office. The deadline for filing with my office is November 1, 2014 at 5:00PM EST, so it is advisable to fill out a Pre-Candidate Questionnaire for the respective academy as soon as possible. Below are the necessary items to complete to apply for a nomination:
1) Complete Application Form. Be sure to complete your essay, attach your photo, and rank the academies in order of your preference. You must rank ALL academies. This is required to maximize your opportunities for nomination. Important! Be sure to include your e-mail address. If you do not have one, you should get permission to use an active e-mail address at your school or one used by a friend. Click here for the Printable Application Form.
2) SAT/ACT Test Scores. Use the recipient code (SAT 1082 ACT 7399) when you register, and copies of your scores will be sent directly to my office. It is advisable that you take the test as soon as possible for purposes of early reporting and optional re-examination.
3) Nomination Recommendation Submission. Three (3) recommendations must be submitted on your behalf. You may wish to ask your principal, counselor, a church leader, coach employer, or someone who knows you well (not a relative) to submit the recommendations. They must send their recommendations directly to my Fort Wayne office.
4) Official High School Transcript (and college transcript, if applicable), reflecting most current course work and grades. Ask your high school to mail an official transcript to my office in Fort Wayne.
