If you would like your health care coverage to start January 1, 2015, you must sign up by December 15

For Vermonters already enrolled in Vermont Health Connect:
- If you do not have any changes to your information from last year, you do not need to do anything. Vermont Health Connect will auto-enroll you in the same plan, and your coverage will renew on January 1, 2015.
- If you have changes to your information (income, employment, personal), you must complete this form and mail to: VERMONT HEALTH CONNECT, 103 SOUTH MAIN STREET, WATERBURY, VT 05671-8100.
- You may visit Vermont Health Connect at any time to estimate your premium tax credit and browse new plans.

For Vermonters signing up for the first time:
- If this is your first time using the health care exchange to sign up for coverage, visit Vermont Health Connect to browse plans, calculate your premium tax credit, and to get covered.
