In order to provide better service to my constituents, I have two district offices in Massachusetts. The offices are located in Peabody and Lynn. My Peabody office is open Monday-Friday 9:00 am to 6:00 pm. My office in Lynn follows the same hours as the City Hall. It is open Monday from 9:00 am to 4:30 pm, Tuesday 9:00 am to 7:00 pm, Thursday 9:00 am to 4:30 pm, and Friday 9:00 am until noon.

In order to expedite your case, kindly fill out this form, and my staff will follow-up with you shortly. It may be necessary to set-up an appointment in order to meet with staff so you can provide us with copies of relevant documents, including all documents and correspondence exchanged between you and the governing federal agency or authority.

According to the Privacy Act of 1974, we require your permission before federal agencies can release information to our office. Please print and fax or mail the Privacy Release Form to my Peabody office so that we have your permission to contact these agencies on your behalf.
