Help with a Federal Agency

One of the greatest honors I have as a U.S. Congressman is helping residents of the 5th District communicate with federal agencies and resolve any difficulties with them.  Below you will find a partial list of agencies with which I can help you.  In order to comply with the provisions of the Privacy Act, before I place any inquiry on your behalf I need a completed Privacy Authorization Form.  Please click on the link for a printable version of the Privacy Authorization Form, and then fax or mail the completed form to the appropriate district office. Constituents residing in Bergen and Passaic Counties should direct the form, along with any questions or concerns, to my Eastern District Office in Glen Rock. Constituents residing in Sussex and Warren Counties should direct the form, along with any questions or concerns, to my Western District Office in Newton.
