Requests must be made to our Allentown office at least four weeks in advance of the date needed. You may submit the form online, or download a copy of the form here and either fax to (610) 434-1844 or mail it to:

Sen. Toomey's office will also provide a certificate in recognition of an individual or group's accomplishment or honor.
