Please complete the required privacy form and provide a copy of any documents which substantiate or are important to the case (please provide a copy of the I-797C, Notice of Action, if available (if applicable)). The form should be completed in detail, signed by the individual concerned and either faxed or mailed to my Seminole District office at:
