Help with a Federal Agency Form

Residents of the 1st Congressional District can contact me for assistance in dealing with Federal Agencies. In many cases, my office is able to facilitate the process involved, gain a fair hearing for your case, and sometimes advocate for a favorable outcome. To begin the process, federal agencies require that I have a written and signed letter when intervening on behalf of a constituent. This requirement falls under the provisions of the Privacy Act of 1974. The letter must be signed by the person directly affected, unless said person is a minor. If your case is medically related, my office may ask for an additional release authorizing access to medical information. Once I have the release, my office can begin to work on your case. In order to better serve you, this form will generate a printable page that you should sign and submit to my Meridian Office by mail or fax.

As I address your concerns, please bear in mind that I cannot force an agency to expedite your case or act in your favor. My office is not able to offer legal advice or recommend an attorney. The rules of the House do not allow me to intervene in, or influence the outcome of, cases that are under the jurisdiction of any court. Nor am I allowed to intervene in matters under the jurisdiction of state or local governments.

Please include all pertinent information and claim numbers in your correspondence—such as:

  • Your Social Security number;
  • VA claim number for a case with Department of Veterans Affairs;
  • Taxpayer identification number (Social Security number, if individual) for an Internal Revenue Service problem, etc.;
  • Your address, home phone number and daytime phone number (if different than home) so that we can obtain any additional information from you that might be necessary;
  • Copies of any related documents or correspondence that you may have from the agency involved;

We must have your signature to proceed with this type of request.

Due to the Privacy Act of 1974 (PL 93-579), Federal and State Agencies are prohibited from releasing information or discussing anything regarding another individual without that person’s written permission. Your signature on the form you print, authorizes me, and/or my representatives, to contact the proper officials on your behalf, discuss the issue, and receive any pertinent information. Your signature also gives me permission to send a copy of this form and any attached letters or supporting documentation to the appropriate agency.
