Way Station Inc. is announcing its Health Home Project. This 30-month pilot in Frederick, Howard and Washington counties will enhance psychiatric rehabilitation programs and assertive community treatment programs to become comprehensive health homes for 1,000 low-income adults with serious mental illness. The two goals of this project are to  improve health and reduce medical costs. The following three objectives will assist Way Station to meet the project goals:  increase access to primary care, improve integration of primary care and behavioral health services, and increase patient participation in care. The Health Home project will include a comprehensive evaluation to measure the clinical effectiveness and cost efficiency of these interventions and assess the project’s success in achieving its goals.
