Introduction

Thank you for your interest in the U.S. Service Academies. As your Representative, each year I have the privilege of nominating a limited number of people to four of the five service academies.

The honor of attending a Service Academy comes with an obligation and commitment to serve in the military for a minimum of five years upon graduation.

Instructions

For individuals interested in applying to receive a nomination please complete the form below and send it in, along with the following items listed, to my Santa Barbara office by the November deadline:

SAT/ACT test scores (as part of the application form below)
Resume
High School Transcripts
Two (2) Letters of Recommendation -

One from a teacher in an academic subject who knows you well.
The second one should be from an adult who knows you from extracurricular activities.

Recommendation by Guidance Counselor/Principal. This form is to be taken to the high school counselor or principal, filled out and sent directly to my Santa Barbara office. The student should not send this form; the counselor or principal must send it.
