* Assistance with Passports *
If you've already applied for your passport and fear that it will not arrive in time for your travels, please contact Rep. Eshoo's District Office immediately for assistance by downloading and faxing the Passport Assistance Request Form to 650-323-3498 or calling 650-323-2984, 408-245-2339 or 831-335-2020 with the information requested in the form. Current information on passports can be found on the State Department's website here.
