Social Security Administration: Description

The Social Security Administration provides benefits and services to people of retirement age and to individuals with disabilities that prevent them from working. Supplemental Security Income benefits are based on financial need, and Social Security Disability benefits are determined through an established eligibility process. Representative Crenshaw can monitor cases on your behalf and follow them through the determination process. By law, he is allowed to check the status of your case and make inquiries on your behalf. He is not, however, permitted to expedite a decision or a hearing or request that your case be approved.

Social Security Administration: Requirements

Before Representative Crenshaw may contact the Social Security Administration on your behalf, he must have your written authorization. Please complete the Privacy Act form, available in Step Two, and fax or mail it to the Jacksonville District office.
