Help With A Federal Agency

If you have a problem involving a federal agency, my staff and I are here to serve you. Caseworkers in my district office are on hand to assist you in dealing with federal agencies including, but not limited to: Social Security, Internal Revenue Service, Veterans Administration, Medicare/Medicaid, Military Issues, Immigration and Naturalization Service, Agriculture and United States Postal issues.

Due to the provisions of the 1974 Privacy Act, we must have your written authorization to make inquires on your behalf. Therefore, you will need to download the Casework Authorization Form. Please make sure the form is as complete as possible. It would be helpful if you would forward a letter to me explaining the specific problem. If you have been in contact with a particular office, please include their names and telephone numbers. Also, be sure to state what you believe would be an appropriate resolution. Photo copies of any documents or correspondence from the agency involved may also be helpful. Please, always keep a copy of your documents or correspondence.
