Help with a Federal Agency

One of the most important things I do as your Representative is help people who are having problems which involve a federal agency. Even though I cannot override the decisions made by these agencies, I can often intervene on a person's behalf to obtain answers to questions, find solutions, or just cut through the red tape.

Please bear in mind that regardless of which agency is involved, I must comply with the provisions of the Privacy Act; therefore, I will need a privacy authorization form from you. In addition, you should also provide a copy of any documents which substantiate or is important to the case. The form should be completed in detail, signed by the individual concerned and either faxed or mailed to my Seminole District office at:
