If you have a problem involving a federal agency, my staff and I are always willing to assist you.

Staff in my district office are on hand to provide you with assistance in dealing with federal agencies.   Please understand that while we can assist you in bringing your case to the attention of the proper officials and guide you through the process, we cannot direct an agency to decide a matter in your favor.

In order for me to make inquires on your behalf, the Federal Privacy Act requires your written authorization.   To do this, please printout the Privacy Release Form on this page, or contact my office at (602) 256-0551 to have one sent to you.   Be sure to include your contact information so that my staff is able to contact you promptly.   Also, please explain what type of assistance you are seeking from my office in the space provided on the form and attach copies of any relevant documents or correspondence. 

Once you have completed this form, either fax it to (602) 257-9103 or mail it to: Office of Congressman Ed Pastor; Attn: Casework; 411 North Central Avenue, Suite 150; Phoenix, AZ 85004. 

If you choose to visit the office to seek assistance, please phone the office and schedule an appointment in advance so that we can best accommodate you.
