Do You Need Additional Help with a VA or Military Issue?

My office is happy to assist you with a VA or Military issue.  To get started,  print and fill out a Privacy Act Release Form and the SF 180 Form.  Once the forms are completed, please fax or mail the completed documents to my Ottawa office.  If you have any questions, feel free to call Casey Gorham in my Ottawa office at (815) 431-9271.
