Help with Federal Agencies

Residents of Illinois’ 11thCongressional District can contact my office for assistance in dealing with a Federal agency. While I cannot override a federal agency’s decision, I can help by cutting through the red tape in answering questions and finding solutions to your problems.

To comply with the provisions of the Privacy Act, you will need to fill out and print a Privacy Act Release Form, which will allow my staff to look into your inquiry with the federal agency. Be sure to include all relevant information and claim numbers in your correspondence including:

  • Social Security Number for cases involving the Social Security Administration;
  • VA claim number for cases involving the Department of Veterans Affairs;
  • Tax ID or Social Security Number for cases involving the Internal Revenue Service;
  • Alien registration number for cases involving the Immigration and Naturalization Service and
  • Copies of any related documents that involves the inquiry agency
