Applying for a New Puerto Rico Birth Certificate

In accordance with Puerto Rico's new birth certificate law, all birth certificates that were issued by the government of Puerto Rico before July 1, 2010 will no longer be valid. U.S. citizens born in Puerto Rico may apply for their birth certificate by mail for a $5.00 fee. This fee will be waived for veterans and individuals over 60.

To request a new birth certificate, please follow the instructions below:

  1. Enclose in an envelope:
    • a completed and signed birth certificate application form.
    • a copy of your unexpired passport or driver's license.
    • a money order or PR Internal Revenue stamp in the amount of $5.00. (Additional copies of your birth certificate can be obtained for $4.00 each.). Money orders must be made out to: "Secretario de Hacienda de Puerto Rico." Personal checks will not be accepted.
    • a self-addressed envelope with paid postage, so that your new birth certificate can be sent to you.
  2. Mail your request to the following address:
