After you have fully pursued your concerns with a federal agency, you may need further assistance from my staff with resolving a problem. To provide assistance, we ask that you complete and submit a privacy form  and send it along with one or two pertinent documents to my office. Please be adviced that your personal information will be held in the strictest confidence.
