If you have a problem involving a federal agency, Jay and his staff always are willing to assist you. Caseworkers are on hand to help you cut through red tape when dealing with federal agencies including: the Social Security Administration, the IRS, the Department of Veterans Affairs, Medicare, the military, visa and passport offices, immigration, disability and federal employment offices. Please understand that while Jay can assist you in bringing your case to the attention of the proper officials and guide you through the process, he cannot direct an agency to decide a matter in your favor.

You may find this contact information helpful if trying to resolve a problem on your own:

Federal government
Washington state
King County
Kitsap County
Snohomish County

If you would like Jay to help resolve your problem, please print and sign the Casework Authorization Form, then mail it to the address below. You will need Adobe Acrobat Reader software to view and print the form; if you do not already have it, you may download the free Adobe Acrobat Reader software here. The reason that you must print out and mail in the form instead of using an online form is because the Federal Privacy Act requires Jay to have your signature before making inquiries on your behalf.

Copies of any documents or correspondence that you have from the agency involved also may be helpful. Please send the signed authorization form and relevant documents to:
