You may purchase an American flag through my office. At your request, I can arrange to have the flag flown over the U.S. Capitol in remembrance of a special occasion or a loved one. After the flag has been flown, you will receive the flag with a certificate of authenticity signed by the Architect of the Capitol in memory of the occasion.

The HTML format requires you to print out the document and hand write the requested information before mailing it in. If you have an older computer or do not have the PDF program, use this format.

The PDF format allows you to type in the information on your computer screen so that when you print it out, everything is filled in. All you have to do is mail it in with your check. The PDF format also automatically computes the cost of the flags you request. We encourage you to use this format whenever possible.
