If you have a problem of a personal nature relating to a federal program, such as Social Security, Veterans benefits, or immigration, please write or call the N.J. District Office nearest you. I will do my best to solve your problem.
If you are unsure which office is closest to you, simply call one of the offices for advice.
Standard procedures in my office require that the constituent either sign and complete a Privacy Release Form or write a letter of request for assistance. The Privacy Release Form is required by Federal Law and grants my office the necessary authority to intervene on your behalf with the appropriate agency. For your convenience, you may download the applicable Privacy Release Form or request one from one of my district offices. Please ensure that you type or print clearly and sign the form. Include in your communication all the pertinent details related to your problem as well as copies of any relevant documentation. Once completed, please mail or fax this form to the closest branch office (I will not be able to accept e-mailed versions of this form).

Dealing with federal agencies is a slow, detailed process and sometimes it takes longer than I would like. However, please know that I will do all in my power to resolve your problem as quickly as possible.
