How can I get a copy of my military records?

The National Personnel Records Center, Military Personnel Records (NPRC-MPR) is the repository of millions of military personnel, health, and medical records of discharged and deceased veterans of all services during the 20th century. (NPRC-MPR) also stores medical treatment records of retirees from all services, as well as records for dependents and other persons treated at naval medical facilities. Information from the records is made available upon written request (with signature and date) to the extent allowed by law. To obtain more information, or to download a copy of the Military Records Request form, please go to NPRC’s website, which is a division of The National Archives. This site also gives information on how to obtain military medals and/or awards for veterans. 



These are general guidelines to obtaining military records. For more information or if your particular problem or question was not addressed and you need additional assistance to get records from your military service, or that of a family member, feel free to contact my office. I will need your written permission to help with this issue.  If you have any questions about this process or military records in general, you can contact my assistant for Veterans matters at my Monticello District Office. 

STEPS IN REQUESTING HELP FROM MY OFFICE

• Confirm that you are a resident of Indiana's 4th Congressional District at Write Your Representative.

• If you have determined that your problem is a federal matter, my office can help!  Download a Privacy Release  Consent Form. Thoroughly complete the form,including the blank lines that ask you to describe your  problem (dates, agency name and what you want my office to do to resolve your problem.) Feel free to use the back of the form or attach a letter with the signed release form.
 
• Sign and date the form and mail or fax it to my Monticello District Office at:
