According to the Privacy Act of 1974, it is required that you sign a Privacy Release Form [Adobe® needed] to allow me to contact a federal agency on your behalf. If your case involves medical issues, I may also need you to sign an Authorization to Release Medical Information in order to be in compliance with the Health Insurance Portability and Accountability Act of 1996 (HIPA). Please print out and sign the Privacy Release Form and Authorization to Release Medical Information (if appropriate) and mail it along with a brief letter explaining your situation. Any additional information that may help my staff and I assist you should be sent along as well.
