Request for letters of congratulations must include:
  • Name, address, phone number, and e-mail address of the person requesting the letter of congratulations
  • Name of the person who has achieved public distinction and the organization he/she represents
  • A detailed description of the accomplishment (e.g., length and type of service; the organization he/she represents; date person was elected, became a citizen, or graduated from high school; etc.)
  • Date of the event at which the person will be honored and/or the date that you need the letter
