Dear House Vendor:

The Debt Collection Improvement Act of 1996 (DCIA) requires that Federal agencies pay recipients by Electronic Funds Transfer (EFT). The objective of the EFT legislation is to take advantage of the improved convenience, reduced cost, and increased security of delivering payments electronically. On September 25, 1998, the U.S. Treasury issued a final rule to implement the requirements of the DCIA.

In compliance with this requirement, the U.S. House of Representatives (House) has changed the way it pays vendors. Instead of issuing checks, the House will now pay you through the Automated Clearing House (ACH) network. Payments will be deposited directly into your checking or savings account. Receiving payments electronically improves cash flow and eliminates the potential for lost or stolen checks.

To assist us in processing your company's payments, please complete the ACH Vendor/ Miscellaneous Payment Enrollment Form. We have completed the House information in Section I. Please complete the Payee/Company Information and Financial Institution Information in Section II. Sign and date the form as indicated in Section III.

Any subsequent changes to your financial institution information must be reported immediately to the House's CAO Office of Accounting.

To comply with Internal Revenue Service reporting requirements, please complete and sign Form W-9, Request for Taxpayer Identification Number and Certification, if the filing requirement applies to you. Specific instructions are also provided.
